Caitlin is an integral part of TCC’s Care Coordination Program. She has been with TCC for six years, during which she launched and led the development of the program. With support from providers, she built the program’s framework, helped define team roles, and guided its growth from a one-person effort to a robust team of six.

Caitlin works closely with the clinic’s most medically complex patients, using a family-led, individualized approach to care. Her focus is on empowering families to navigate the healthcare system with confidence and clarity.

Over the years, Caitlin has supported hundreds of families, forming deep, meaningful relationships and becoming a vital part of many children’s healthcare journeys. She is passionate about meeting families where they are, creating affirming spaces where their voices are heard, and equipping them with the tools and support they need to thrive.
