Choose Visit Type
In-Clinic Visit
- All Well-Child checks must be scheduled as an in-clinic visit
- For patients younger than 2 months, please call your location of care
Virtual Home Visit
- Virtual visits may be scheduled for any problem or illness visit
- Well-Child / Physicals must be scheduled as an in-clinic visit to meet your insurance requirements. (See In-Clinic Visit)
- COVID-19 testing – If you are needing a COVID test, please call and speak to one of our advice nurses to schedule a test without a visit with your provider.
To avoid a No Show/Late Cancelation charge of $25.00, please notify the clinic 24 hours prior to your appointment if you need to reschedule or cancel.
